Skilled Nursing Facilities

Concierge Services: We recognize that all post-acute facilities are different. We therefore listen and cater to any specific needs, but as a whole can help via:

Readmissions & length of stay by way of additional clinical oversight when attending physicians are not present on-site.

Increasing resident satisfaction, improve resident outcomes, improve CMS star ratings.

Providing a visible presence at facilities: our specialists can attend regular interdisciplinary meetings and provide valuable input in resident care coordination.

Providing accurate clinical documentation to fully encapsulate the medical complexity of residents receiving therapy services.

Increasing revenue for facilities.

Pain Management services: Including pharmacological adjustments (including opioid management if requested) and injections. We can significantly reduce the burden of primary attending physicians and medical directors, letting them focus purely on medical management. Let our specialists who have had extensive training in non-interventional pain management handle all your residents’ pain issues.

Close collaboration with therapy teams to recommend changes in services, lengths of stay, etc. We can perform peer to peer reviews with payer sources to help increase resident lengths of stays if needed for therapy. Our rehab specialists can also write prescriptions on-site for necessary durable medical equipment, orthotics, and prosthetics to help residents improve their function without any delay!

Frequently Asked Questions

There is no cost to facilities! We bill payers directly and do not interfere with facility reimbursement. As consultants, we also do not interfere with primary care physician billing.
We serve as consultants for your residents who are receiving therapy services. All physicians are trained to provide basic medical care, but we will defer those decisions to your primary care attending physicians already in-house. However, we can provide assistance with pain management and therapy optimization, and can even perform time-consuming peer to peer reviews for residents on their behalf using our rehab knowledge. We operate under a model of collaboration and communication with your attending physicians, helping free them to focus on their jobs as the main providers of medical care in your facility.
Yes, CMS currently requires licensed PM&R physicians to staff acute inpatient rehabilitation hospitals. However, the types of patients who were appropriate for acute rehab 10-20 years ago are now heading to SNFs, with the same complex medical issues and major physical impairments. Today more than ever, physicians with specialized knowledge of rehabilitation are necessary to ensure that SNF residents get the care they require and deserve. Imagine a post-acute facility without any medical rehabilitation oversight; residents would be at increased risk for decompensation, wounds, infection, falls, intractable pain, and hospital readmission. Let us use our expertise to help your facility prevent these and stand above the rest!
Our clinicians will see all your residents who are receiving physical, occupational, and/or speech therapy. We also accept all insurances.
Absolutely. Adding one of our consultant physicians to your facility will generate more accurate clinical documentation which can encapsulate the entire spectrum of appropriate medical diagnoses. This is important to ensure your facility gets the appropriate reimbursement (via compliance with PDPM and increasing case mix index) for caring for the complex residents of today. Our documentation will also help with risk reduction and potential audit defense.
